WAIVER, INDEMNITY AND CONSENT Release and Waiver: By applying for membership, I undertake to be bound by the rules of Matsimela Ladies Clinic. I warrant that I am in good health and physically fit to participate in the training. I agree to comply with the rules, conditions, and regulations for this training or learning, which include the payment of the membership fee in advance which is non refundable.
I hereby release and discharge the coaches or trainers and/or directors of the Matsimela Ladies Clinic, all sponsors, volunteer groups, medical personnel, and all local authorities, from any loss or damage, however caused, arising from my participation in the training, including pre-training and post-training activities. This waiver applies to my executors, my heirs, administrators, assigns and myself.
I also grant my permission to the Matsimela Ladies Clinic or its agents, in terms of Section 51 of the Electronic Communications Transactions Act 25 of 2002 and the Protection of Personal Information Act 4 of 2013 (POPIA), to use my name, race information and any photographs, video tapes, broadcast and/or telecast footage in which I may appear, for whatever use, at no charge.
